The intake process, step by step.


Prospective patients begin with a consultation request — not an open booking link. Submissions are personally reviewed by the physician. When the clinical profile and fit are appropriate, a comprehensive evaluation is scheduled. The process is designed to feel thoughtful and selective — not bureaucratic.

Submit a Consultation Request

Tell us briefly about what brings you here, your timeline, and your contact preferences. The form below takes a few minutes. There is no charge or commitment at this stage.

Physician Review

Dr. Crawford personally reviews each request. This is a small, relationship-based practice — not a scheduling queue — so we want to confirm that the practice is the right fit for what you need before any time or fees are committed.

A Brief Reply & Next Steps

You'll hear back within a few business days. If a comprehensive evaluation is appropriate, you'll be invited to schedule one and complete a small set of intake materials securely in advance.

Initial Evaluation + 30-Day Onboarding

A 60–90 minute telehealth evaluation followed by 30 days of onboarding care. You’ll receive individualized clinical impressions, treatment recommendations, and early treatment support without a separate membership charge during the onboarding period.

Optional Membership Transition

If fit is mutual and you wish to continue, you’ll select a membership tier and authorize recurring payment. Your first membership charge occurs after the 30-day onboarding period ends.

The initial evaluation is unhurried — 60 to 90 minutes — by design. Diagnostic clarity takes time, and rushing it doesn't serve you. You can expect a careful review of:

Toward the close of the visit, Dr. Crawford will share clinical impressions and treatment recommendations. The $650 fee also includes 30 days of onboarding care. If continued care remains appropriate and mutually desired, you’ll select a membership tier and recurring billing will begin only after onboarding ends.

A Quiet, Private Space

Telehealth works best from somewhere you can speak openly without interruption. A door that closes is helpful.

Stable Internet & a Camera

A laptop, tablet, or phone with a working camera and microphone. We'll send the secure video link in advance.

A Brief Symptom Timeline

Even rough notes on when concerns began, what's helped, and what hasn't — it speeds the conversation along.

Current & Past Medications

A simple list of current medications and any psychiatric medications previously tried, along with dose and effect if you remember.

Relevant Records (if any)

Recent labs, prior evaluations, or therapy summaries are helpful when available — but never required to begin.

Cancellation Policy

Plans change.


Initial Comprehensive Evaluation

We understand that plans change. Appointment modifications with at least 48 hours' notice may be accommodated. Changes requested within 48 hours of the scheduled visit may require a $200 rescheduling fee before a new appointment is confirmed. Appointments missed without prior notice may be treated as forfeited.

Membership Visits

Reasonable advance notice is appreciated when rescheduling. Repeated late cancellations or missed appointments without notice may result in additional fees or affect scheduling priority.

Scope of Practice

If you are in crisis, this is not the right place to start.

Premier Psychiatry & Wellness is an outpatient practice. It is not appropriate for — and does not provide care for — psychiatric emergencies, active suicidality requiring a higher level of care, acute psychosis, or severe untreated substance use disorder requiring intensive support.
